Multiple Sclerosis

Presentation

After being diagnosed with multiple sclerosis (MS), a patient was hospitalized for stroke-like symptoms.

Following discharge from the hospital, the patient received minimal education or instruction and was scheduled for an outpatient neurology appointment two months later. This delay in treatment created immense concerns about potential disease progression. This left the employee feeling anxious and distressed without a treatment provider or a clear plan in place.

PHM Actions

The PHM Care Team offered tailored education and treatment decision guidance during an uncertain time.

The PHM Care Team responded with a high-touch emotional, clinical, and personalized support approach. This comprehensive approach made sure care was timely and went beyond standard practice.

The PHM Care Team collected and reviewed the employee’s medical records in order to create a clinical timeline and facilitate an expedited consultation with an MS expert neurologist within one week of the employee engaging with PHM. To guide this consultation, the care team developed a list of questions to address key treatment considerations and provided a disease-specific research report with the latest MS scientific research and drug developments to help the employee consider treatment options.

Following the consultation, the PHM Care Team provided decision support to help the employee initiate medication immediately, based on expert input and thorough review of the treatment landscape. Without this support, the employee would have been left uncertain about how to proceed. The PHM-identified local MS expert confirmed this treatment decision, and the employee chose to continue with this provider.

Outcome

The PHM Approach bridged the gap in care following hospitalization with clinical guidance and treatment support, preventing complications.

The personalized clinical and research guidance instilled confidence in the employee’s treatment decisions, even with the pressure of making critical health choices in a short timeframe. The identification of top local providers set the employee up for ongoing care with a qualified MS expert at a leading MS center. Above all, PHM’s emotional support and assistance with logistical coordination saved the employee valuable time during a highly stressful period.

The PHM Difference

PHM’s high-touch, personalized approach provided emotional, clinical, and research support during a time of uncertainty, making timely treatment possible while exceeding the level of care the employee would typically receive.
